Fly wheel resurface and clutch fork

Two questions:
M21 Trans...

Flywheel:
Is there a minimum thickness for flywheels. Mine has been resurfaced a few times and I am wondering if there is a minimum thickness so that everything aligns/works properly? I am having troubles with the clutch chattering and am considering having it resurfaced again.

Clutch Fork:
My clutch fork is not that old but has flat spots on the fork finger surfaces that press against the throw out bearing? Is that normal or does the wear indicate an adjustment problem? Should I replace the fork? I have read that the pads should be rounded.

Thanks,
Andy
