Try the local junkyards - on the back of CCC equipted GM 8 cylinders, with 200R4's, you'll find the bracket.
Should also find your cover, dipstick+tube, all, for I'd guess under $50.
Ask them for the connector, too!

You're in luck! You won't need a kickdown bracket.
You will need a TV cable, and a proper cable bracket for your carb.
I would try E--bay for the bracket. I bought one for my holley that way.
You may have trouble getting a stock 2004R dip stick tube to fit. I bought a Lokar flexible unit that was more expensive but fits and works well.
Also, be careful with the transmission cooler lines. Make sure the threads to the trans are correct since the 2004r is a metric unit. I'm not sure if this is an issue since I used the fittings that were on my 2004r core for my swap. If you are adding a cooler, a cheap solution is to bend up some new lines yourself.
My biggest problem with a floor shift was getting the proper cable bracket for the trans. I used one from a buick GN that works perfect. My shifter is a B&M unit so I can't help you if you have a stock console.

Could you post a picture of the trans linkage bracket that you used? I am trying to help Charles get squared away on his swap, and need to know which type I should grab from the salvage yard for him.
