I would also suggest going to your local shop. I found a local guy that has done work to my Olds a few times. He always does great work, and he has prices that can't be beat.
As far as your other questions go...
IMHO, I wouldn't go over 2.5 inch with a small block. I might also add the X pipe (just for fun) since you have some other aftermarket parts on the engine.

Here in the DC are, very few muffler shops do custom work anymore. In fact, with stainless and aluminized factory exhausts, most concentrate on catalytic converter replacements (and brake jobs!).
I like Pypes Exhaust pre-bent sets. They have 2 1/2" sets with or without an X pipe. Their sets are designed for headers, but they also sell add-on down pipes to mate to factory manifolds.

I put on a flowmaster 2 1/2 mandrel bent kit, h pipe, and with super 44s on my 72 cutlass. the kit is designed for headers, cost a little more than a shop bending up a system, and sounds good as well. the only issues were the driveshaft nicking the h pipe with all the weight off the suspension, and the clamps were a little cheesy
