Ken, the 330 and 350 used the same manifolds. There were a couple differences in appearance between the years. Are you looking for a specific year to keep your car looking correct or just one that is crack free? I've got several of these and am sure others here have extras too. I didn't notice if you have your location under your name, but if you don't please add it. If your on the East Coast you'll be better off getting one from one of the guys there. If your on the West Coast or even western third of the country I could help with one.

Now that I made my post I can see your in Ohio. So skip the location, but please add if you want the appearance for any specific year or just one that's crack free and not badly pitted.

The manifold is for 64 cutlass, rebuilding HO 330. Live in cleve, Ohio area.

 

The drivers side 1964's I've seen don't have the large 1 cast into them. But they do have the same casting number, 390001, just like was used into the 1970's 350's. Do you want one that is a 1964 manifold or one that will fit and function just fine?
