Rear window keeps popping off it's ...connection points

Hi. Not sure if this is the correct sub-section for a window question but...

I have a '72 98 and the drivers' rear window has popped off it's regulator/rails/lower plastic round connection a few times. Thinking I need a new ...white plastic 1" round connection point for the bottom "nipple" of the glass. Wondering if anyone else has had this happen, or purchased a new white plastic disc to solve the problem. (See pics).

Is it possible that the metal regulator that the nylon "washer" rides in has been bent a little or is maladjusted? It looks like the lower end of the regulator can be moved by loosening the bolt on the bottom and moving it a little one direction or the other. Replacing the nylon part would not hurt but may not be necessary.
