I think I would just wire brush it and spray on (or brush) some of that rust converter stuff. I'll bet most older original cars look like that under the headliner. I don't think it would ever be a problem..... (I could be wrong.....)

^x2^ I suspect nearly all classics demonstrate this surface rust. I had a 1962 Dodge Dart in 1973 I replaced the headliner & it looked about the same. I like what Greg suggested.

Got a good 3M wheel to knock a lot of the rust off and then hit it with a can of EAstwood Platinum that I had open and laying around.
I know that color doesn’t really matter on the inside, but just the IDEA of painting the ceiling black here in Florida made me start to sweat.
