After re-installing the engine, I discovered a huge vacuum leak. This is what I have remaining. Its the power brake booster vacuum check valve. It appears to be missing something. I don't know:
1. if a cover got knocked off or
2. there was a grommet and fitting
Dorman shows two variations. My question is: What was originally used on a 1964 Olds Starfire Power Brake Booster ?

Your rubber grommet appears to be there under the check valve and YES, the insides/center part of the check valve has fallen out.....massive vacuum leak.
I don't know exactly what your original check valve looked like. I have a original '67 booster from a Cutlass wagon and it had the larger style "straight in" check valve instead of the 90 degree style you have ('67 also has a much larger hole and grommet in the face of the booster where the larger "straight in" style check valve goes).
Pic below shows the '67 style larger "straight in" type check valve. Looks like your grommet is the smaller diameter style made for the smaller 90 degree check valve (like you have now...but missing parts).

The difference in the Dorman parts is whether the car has Moraine or Bendix power brake booster. Your car has a Moraine booster so if you can find a check valve similar to what you have (minus the missing guts) it will work fine. And Olds used Moraine boosters on many years and models.
Here are two new check valves I have. The valve on the left is from Inline. The valve on the right is from a local NAPA store.
