I don't recall one ever being built by a manufacturer. Some have taken a four barrel manifold and made an adapter plate that bolts to the intake and the blower bolts to the adapter. Or use a Paxton type blower that fits over the entire carb. Like the Ford T-bird or Studebaker Avante
The only SBO blower intakes I have ever seen have been fabricated by taking one of the Offy 360 dual quad intakes, milling the carb flanges parallel to the crank centerline, and welding a thick aluminum plate to the top. This one is a BBO, but you get the idea.
on face book some guy has a bunch of intakes and on the bottom of the pile there is a blower intake..not sure if big or small..but he wants to sell..i dont know them or anything about them..it looks like a small block , but then again..it looks like a 455..its sitting backwards so i cant get a good look

Unfortunately it is still an Offy intake with a blower adapter welded to it. It isn't even a dual quad, it started life as a single quad intake. Compare the second photo with the blower intake, particularly at the rear. Yes, it is a BBO intake. The SBO version of the Offy has differently shaped runners at the back (third photo).
