Dumb steering wheel question

Wen I first bought my car the steering wheel wasn't lined up with the wheels being straight. (see pic)

So I straightened the wheels, pulled the wheel off, and put it back on perfectly centered (with the bars sideways) After I drove it to the store and back it went back to the exact same way it was in the first picture.

Am I missing something stupid?

check the rag joint on the shaft it might not be seated right and allowing it
move. (NOT A GOOD THING) does it feel lose or non responsive to wheel
motion?

Wheel motion seems normal, at least to me anyways. I will check out the rag joint this weekend.

Did you check the mechanical condition of your steering box? There are adjustments to eliminate free play, but I suspect the worm gear may be worn out and the drive gear is jumping teeth when it arrives at its mechanical limits.
