I have a factory 69 Cutlass/442 4 core support. Big saddles, tabs for the shroud etc. I get that it may have come from a car with a/c, but what was the other criteria? Just a lower rear end?

I know certain rear axle ratios required a big radiator. Trailer tow options, A/C, and obviously selecting the heavy duty cooling option would get the big radiator and other components. I have no doubt Joe or some other people would have factory literature showing all the details.
There was no such thing as a "trailer tow option". Olds offered a trailer wiring harness but not a tow hitch. The trailer towing guide told you how to select options for towing trailers of various weights. As for the use of the four row core support, the PIM is very clear. Core support P/N 402707 was used with the 350 engine when RPO V01 or Y72 was selected. It was used with the 400 engine when RPO C60, V01, or Y72 was selected. All other applications got core support P/N 402706, which has the saddles for the two and three row radiator end tanks. C60 was air conditioning, V01 was HD radiator only (four row) required when either G88 (3.91) or G89 (4.33) gears were ordered (and yes, 4.33s were installed at the factory on W30 and W31 cars with manual trans that year). Y72 is HD cooling equipment, which includes the V01 HD radiator in addition to the clutch fan and the 55 amp alternator.

Ok well I’m guessing it wasn’t original to the car. It was in a 69 Cutlass automatic with a 350 2bbl. No a/c. 2.78 rear.
It could have been if the original buyer had ordered V01 or Y72. I can't see why they would have, but it isn't impossible that it's factory. It's also possible that the front end was replaced with wrecking yard parts after an accident. I've found red inner fenders spray painted black on cars like this.

Did Olds offer a tie package in the later years? I would have sworn I have seen a tow package in Oldsmobile literature.
Joe, you need to share some pictures of your library, man cave, or wherever you keep all your books and manuals. It would give all us little people something to aspire to.
