Welcome to the site, I moved your post to get a better response. Hopefully someone familiar with your model will chime in soon. In the meantime post a proper intro in the newbie section. We do like pictures.

Use the 371 flywheel , lower flywheel housing , and starter . You may have to grind the inside of the upper flywheel housing on the block a little for flywheel clearance .
use the 394 motor and trans mounts . If your lucky they will bolt to the frame . If not you may have to drill some new holes . You may have to modify the accelerator linkage and the linkage from trans to carb . The 58 carb may be the better choice here .
The exhaust pipes will have to be fabricated , a local muffler shop with a pipe bender should be able to handle this. Make sure you have clearance for the fan, the 64 fan sits lower. Fabricate a generator bracket or change your wiring over to use an allternator, your choice. If you have vacuum wipers , you may have to change over to electric.

I wouldn't use a slim Jim if I could get the jetaway to bolt up, Slim Jim's are notorious for being problematic. You may need to re drill for a larger pilot bearing if it's not already done. The 371 intake wont fit because of the higher deck height of the 394, makes it to wide for the ports to line up on the intake. So use what you have on the 394 top and adjust the linkage to fit the difference in deck height.... Tedd.

63-64 394 front mounts are a little different than 49-62 Rockets. Not sure what you might get into there. I don't know for sure if the 58 371 front cover and water pump will transfer to a 63-64 block either, but it if will that will make your job simpler. It'll also make it easier to find a water pump down the road.
