Been a while since I was researching, but Chris at CK Performance and Lonnie at Extreme Automatics were the go-to guys for 2004r transmissions when I was looking. I ended up buying a stage 2 4L60e built by Lonnie and it has been very good behind my 6.0 LS. A bit of a heads-up though, you will need to be patient as he is a busy guy.

Do not use Art Carr transmission (zombie company now, like Mondello). My buddy got a California Performance Transmission (CPT - Art Carr's new company) 200-4r but I'm not impressed with the calibration.
I got a stage 2 from Lonnie @ Extreme. The process went well and I think it's well built. The governor was set way too high on first install, so I had to pull it and mail it to them so they could put different weights on it. Their converter guy - R&R - I'm less happy with. His timelines are getting worse, the stall on the unit is about 1,000 rpm higher than I requested, and his prices are high.
So as of two years ago, I'd say either CK or Extreme, and just get a converter from PATC.
