chasing cold start misfire

When the car starts up cold, and is sitting on the highest lobe of the fast idle cam, the car has a noticible misfire. Once the idle comes down a bit, and it begins to warm up, it goes away. The miss isn't present when in D or R, or when the car is moving.

I have replaced it no particular order, just as general tuneup:
Plugs
Wires
Cap
Rotor
Points
Rebuilt carb

All of these have contributed to the car running significantly better then when I first received it, but haven't resolved this particular issue.

It could be a sticky lifter, fouled plug, choke issue, etc...
