I have ran into the exact same issue. I found a local shop to do the foam and burlap and put on my covers for 175 so that was cheaper than I found the rear foam for the hard tops. Call some local upholstery shops and you should be good.

From the factory, foam was preformed. It is not common to find it reproduced and if you do find it, it will be costly and may need to be trimmed anyway.
I think the back seat back was just stuffed with cotton batting (no foam). It can be replaced with foam though.
To replace, I would suggest just get sheet foam and new burlap from a fabrics / upholstery store and cut to fit.
Use your original as a template of how much you need. The medium density is best for the bottoms, soft for the backs.
If your foam is 'okay' but just a bit compressed, reuse it and put a 1" thick layer of new foam on top of it to firm it up and fill out the covers.

I had to do that on my 71 Supreme front seat and it worked out great. I paid $25 for the 1" of foam sheet I needed.
Is your original material falling apart?

I got 2" foam (green) from a fabric supply: "Joann's," and "Hancock". I waited until they had a sell for 40-50% off on the foam. It was about $40 after the sale. One sold it by the lineal foot, the other in a sheet that was plenty for the bottoms of either the back, or front bench. But my padding was about shot/gone. I also, beef up the front bottom bench with 3/4"fabric backed and hog ringed that tight across the springs.
