It's a 5 x 4 3/4" bolt circle and the axle is unique to the 1964 J88, so good luck. Your post isn't clear - did you bend the axle shaft or the whole axle housing? If it's the axle shaft, most of the after market custom axle vendors (Strange, Mark Williams, etc) can make one for a reasonable price. If the housing is bent, a good axle specialty house can straighten it and even replace the tubes if necessary.
